These terms govern use of the Codavia Health website and platform. By accessing the site or using the service, you agree to them. If you are agreeing on behalf of an organization, you confirm you have authority to bind that organization.

1. The service

Codavia Health provides a cloud-based electronic health record platform, including clinical documentation, orders, e-prescribing, laboratory and imaging workflows, billing and revenue cycle features, a patient portal, and an AI suite comprising an ambient scribe, clinical decision support and automated coding assistance.

2. Clinical responsibility

Codavia Health is a documentation and workflow tool, not a provider of medical care or medical advice. All AI-generated output — draft notes, coding suggestions and decision-support prompts — is advisory and must be reviewed by a qualified clinician before it is relied on, signed or submitted. The treating clinician remains solely responsible for clinical judgment, the accuracy of the record, and the correctness of any submitted code or claim.

3. Accounts and access

  • You are responsible for the accuracy of account information and for all activity under your credentials.
  • You must not share credentials. Each user requires their own account so audit logging remains meaningful.
  • You must notify us promptly of any suspected unauthorized access.

4. PHI and the BAA

Where the service processes Protected Health Information, a Business Associate Agreement governs that processing and takes precedence over these terms in the event of any conflict. A BAA is signed before any PHI is shared. Our privacy practices are described in the privacy policy.

5. Fees

Pricing is per rendering provider, per month, as published on the pricing page: Essentials $149, Professional $349 including a 250-note AI Scribe allowance per provider per month, and Enterprise from $599. Additional scribe notes are billed at $0.90 each. Front desk, nursing and billing seats are included at no additional charge. There is no implementation fee for pilot practices.

6. Acceptable use

You agree not to: use the service unlawfully or in violation of HIPAA or other applicable healthcare regulation; attempt to gain unauthorized access to the platform or another organization’s data; reverse engineer or resell the service; or use it in a way that degrades platform availability for others.

7. Your data

You retain all rights in the clinical and business data you and your users enter. We claim no ownership of it. On termination, data is returned or destroyed in accordance with the BAA and any applicable data-return terms. Export format and window: [ confirm export terms ]

8. Availability

9. Warranties and disclaimers

Except as expressly stated, the service is provided “as is”. We disclaim implied warranties of merchantability, fitness for a particular purpose and non-infringement to the extent permitted by law. We do not warrant that AI-generated documentation or coding output will be error-free — clinician review is required.

10. Limitation of liability
